The Mars Climate Orbiter
The loss of the Mars Climate Orbiter is often described as a simple mistake, but it also reveals broader failures of communication and systems engineering. NASA lost contact with the spacecraft in September 1999 as it approached Mars. One engineering team generated thruster data using English units, while another part of the system expected metric units. The resulting mismatch caused the spacecraft’s calculated path to differ from its actual path. The error remained undetected despite unexpected navigation data and repeated course corrections. The ethical question is whether responsibility belongs to the person who failed to specify or convert the units, or to the organization that lacked adequate checking, testing, and communication procedures. This case illustrates why complex projects should never depend on the assumption that another team has interpreted technical information correctly.
