The Columbia Disaster

The Columbia disaster illuminates the ethical responsibilities of engineers when safety concerns are dismissed because an organization has become accustomed to recurring technical problems. During the 2003 launch of the space shuttle Columbia, a piece of insulating foam broke off from the external fuel tank and struck the shuttle’s wing. Foam shedding had happened on many previous launches, so NASA had come to treat it as an acceptable or routine problem. While Columbia was still in orbit, engineer Rodney Rocha and others wanted additional images to determine whether the wing had been seriously damaged, but the request was rejected. The Columbia later broke apart during re-entry, killing all seven astronauts aboard. The central ethical question is what engineers should do when managers dismiss technical uncertainty in a decision that may affect human life. This case highlights the danger of “normalization of deviance,” where repeated acceptance of small failures makes an organization less responsive to a serious risk.
